Did the nurse make an ASSUMPTION that the blank form would have been filled out by the patient, since it was in his file, and then advise not to resuscitate? Pretty big ass-umption.

It’s an insane assumption, since there’s no way the patient could have put the blank form in his own file, and no reason he would. I’m thinking this is not really a mistake, but that DNR is the default assumption at all times, unless there’s a family member present to insist on the patient’s receiving care.
