I believe Howie Carr bestowed the moniker on the Kennedy tot. Reference, I believe, is to an old Dickey Lee song by that name.

He got that name from Howie Carr and is based on the 1970s Clarence Carter song "Patches". In that song, a father on his deathbed pleads with his son to "carry the family through."
