He's probably referring to the "medical home" concept in the bill. Best analogy I can give you is that seniors on medicare will have a "medical home" for primary care (similar to how HMOs work), with all speciality work requiring pre-approval by the medical home. It will slow care and put gov't officials in the drivers seat on oversight of care plans.
By the way, the plan also replaces MDs for hospice patients with PAs.
