The pilot of an Apache gunship, who flew to the rescue of U.S. soldiers nearly overrun at a remote outpost in Afghanistan last weekend, told ABC News today that he had "never seen that large of a force" attacking coalition troops in Afghanistan.
By the time Warrant Officer Ross Lewellen and Warrant Officer Chad Bardwell arrived over the embattled outpost, dubbed Camp Keating, it was in flames and dozens of insurgents could be seen on the camp's perimeter.
When the battle was over and the fire extinguished, many who survived had nothing left "except the clothes off their backs and the weapons in their hands," one soldier told ABC News.

Every single one of these needs to be answered by a very large raid on the tribal areas that are the sanctuaries. We know where they are. We know who they are. Every time one of these guys leaves for jihad he needs to be thinking he's coming back to a smoking hole or not at all.

We will not do this but a more “ruthless or nasty” approach might be called for.
Don't want to hear from the just “nukem” crowd or any other “magic weapon” approach.
Starting to starve or otherwise eliminate the population by other means and letting then know that we will not stop till THEY kill the Taliban or AL Queda might produce results.

So we either get out or prepare to spends decades, lives and money there.
The camp is located at the base of two steep mountains, allowing the enemy to fire down on the camp with a powerful .50 caliber machine gun and other heavy guns.
Stupid. The only way to maintain security in a situation like that is if you OWN the mountains.

....from May of 1959 to Easter of 1972 the PAVN was able to put ever larger formations in the field....(I stole the “getting it together” phrase from Dale Dye when he was at Hue)
Is that why the kill ratio was so skewed? toward the end it reached over 100 to one. - Perhaps large groups made it easier to generate large kills?

AC-130s operate only at night; this attack kicked off at dawn and continued throughout the day. Avoiding “Spectre” and “Spooky” was clearly part of the plan.
