86,000 is not a large number. There are 3140 counties in this country. 86,000 works out to be .54 persons per county.

86,000 additional deaths would be an increase of <3% of the death rate. Probably lower than that, because many of the people who die of H1N1 will be people who were medically fragile and would have died of something else.
