It happens; about 1 in 100,000 individuals have two appendices.
Or, as a surgeon friend pointed out to me, if the appendix had burst prior to the first surgery, the surgery would have been rather challenging, and if there was any stub left (always a possibility in such cases, which are a big mess), it can re-perforate at some later date.
Many things could have happened, including rank incompetence, but not necessarily so.
