IIRC, yes, there is a penalty. $500 per form if you lie, $100 if you just don’t answer. They may have added a zero to those numbers though.

Don’t lie on federal forms. Ever.
Getting fined for not answering, however, gives you a 4th Amendment defense - which I hope we’re all looking forward to exercising.

Just in case some reading your post wants a reference:

Amendment IV

The right of the people to be secure in their persons, houses, papers, and effects, against unreasonable searches and seizures, shall not be violated, and no warrants shall issue, but upon probable cause, supported by oath or affirmation, and particularly describing the place to be searched, and the persons or things to be seized.
