Fold the top third downward. Fold the bottom third upward so it's over the first fold.
Fold the left quarter inward. Fold the right quarter inward so that the edges of these two folds touch (or nearly so).
The paper is then folded at the middle, where the two previous folds meet. It appears that this fold is done so that the two previous folds will be inside, however I can't say for certain that this final fold is thus done due to shadowing or the possibility that the photographer flattened this crease.
I think an image of the outside of the document is just as important as the inside for 'forensic' purposes. This document remained in the folded state for quite some time, so there will be a difference in color due to oxidation. Only two faces should show a slight difference in color, those being the faces exposed when the thing was folded. Also, there's likely scuffs/wear along the exposed edges while it was in its folded state for so long.
