Things like that happen all the time. I remember a bridge in California that was built backwards. There were dozens of opportunities to recognize the error before the concrete was poured but it happened anyway. It was a rural bridge over a freeway and wouldn’t have been a problem except it only had one sidewalk and it ended up on the wrong side. We realigned the road on each side of the bridge to match the deck and now people drive across it with no idea of the headaches it caused.
