ANY prior relationship between an attorney and a judge before which a case is tried is cause for disclosure.

I'm sorry to be blunt, but that's just flat-out wrong. There are numerous categories of "relationships" between judge's and attorneys, and many, if not most, of them require no disclosure. That's how it works.

If you don't believe me, go down the the local courthouse, federal or state, and ask around.

It's not that the judge knew an attorney. The judge knew and was friends with a principle litigant of a case in which she was acting as an appellate judge. At minimum, she should have disclosed the relationship giving plaintiff's counsel the opportunity to object. I think most lawyers would agree that as a practical matter, she should have recused herself.
