A new theory is that it could have been an oxygen tank. A qantas 747-400 had an oxygen tank burst in flight, and the tank (about the size and shape of a scuba tank) flew up through the floor from the cargo hold and hit a door handle and the ceiling.

CANBERRA, Australia — An air safety investigator confirmed Wednesday that an exploding oxygen cylinder in the hold of a packed Qantas jetliner caused the large hole in the fuselage that forced the plane to make an emergency landing.
