It is not a fill-in-the-blanks form, rather it is a wide-ranging set of questions.

Unless I missed it, though, it doesn't seem to call for the disclosure of past or current illegal drug usage or abuse of prescription medications (there is a question that asks about past arrests, but that's a separate issue, and broad questions about background contacts and associations, which is tangentially related). No doubt an explicit disclosure of drug usage would disqualify many people whose help is eagerly sought by the incoming Obama regime...
