Okay, we went down to the local store and they REFUSED to give us a form. They called the manager (of the day), and he refused to give us a form. The lady said that they have been getting lots of complaints from Republicans (said in a snotty manner)!!

Also, we've left a voice mail for the lady we spoke to previously and asked her to please send us a copy of the form.
