Anything is possible, but I've done some checking for you and concluded this is unlikely. There has long been a standard U.S. birth certificate form, and although it has undergone changes over the years, from what is shown in the govt report on it, there has never been a religion field.
A state could decided to add it, of course. I just dont think it's very likely in a state like HI.

BESIDES THE VISUAL PROOF, CONFIRMATION HAS BEEN RECEIVED FROM THE HAWAII STATE DEPARTMENT OF HEALTH (HSDH) THAT:
1. Certified copies of birth certificates are NEVER sent electronically. Copies can be requested by qualified individuals in person, by mail, or on line; and the Vital Records (VR) office will MAIL the Certification of Live Birth within 4 to 6 weeks.
2. Certifications of Live Birth will ALWAYS include an embossed seal.
I just looked at the comparisons, HUGH difference!
Obamas says Date Filed By Registrar and the other one says Date Accepted By State Registrar
I think the borders look different. I can see a line or overlap on the corners of Obamas and not on the other.
Obamas doesnt have the embossed seal, the other one does.
Also the type on Obamas look computer generated. They just dont look the same.
Both say: OHSM 1.1 (Rev. 11/01) LASER, likely meaning that this form was revised in Nov. 2001 for use with laser printers, AND therefore that both of these certifications theoretically would have been produced by the VR office in Nov. 2001 or later. Therefore, it doesnt mater if they were born years apart, Obamas certificate was revised and should have an embossed seal.
Hmm. It's been a long time since I've looked at my birth certificate. I was baptised at the hospital by a prominent national figure in the church who was passing through and looked at some sort of information for persons of my mother's denomination.
It is POSSIBLE that he went door to door around the ward asking the mothers their religion, but I think he looked over something else. He wasn't called there by my family, I know that.
