Historically, the longest-serving apostle has always been chosen to succeed the deceased prophet. But it doesn’t necessarily HAVE to be that way. It depends what the Quorum decides, under inspiration.

Realistically, yes, it’s 99.9999% likely that Monson will be the next President.
