When did he fill out the questionnaire?

My guess, and this is only a guess is that whoever had the power of attorney, or next of kin filled out some kind of questionaire.

I questioned the risky behaviour because of the dust up last year on some college campuses to relax or delete the ban on homosexuals donating blood.

In Canada, donors are given a confidential form where they can indicate that their blood shouldn’t be used.

This allows “high risk” individuals to donate, along with their friends, without anyone knowing about their “risky behaviour”. That way, homosexuals can donate blood — but, it doesn’t get used. (Provided that they’re even a bit honest.)
