It apears the application/form was revised 07/01/2003. Its possible that the group mentioned is a different version of the modern day Minutemen. That would be Robert DePugh's Minutemen, who were something like the military wing of the John Birch Society in the 1960s.

I wouldn’t be surprised if the original form was refering to the subversives of the sixties. When the form came up for revision, the geniuses doing the revision thought they meant the current Minute Men, and left it in. Most govt HRD types are liberals.
