Just a question for those more familiar — the document the author is upset about... could it have possible been a sample/training document to show folks how to fill out the form, that was accidentally submitted instead of (or inadvertently substituted for) the real certification?
I just like to exhaust the possibilities of incompetence before I start discussing malice.
Stuff like that happened to me. When I was a local TV weather forecaster, I had to break in a new guy on the graphics computers. Doppler radar is meaningless unless you have a good base-map to show where all these colorful splotches happening in the real world.
I was showing him how to put labels on the base-map and demonstrated by relabeling the "Gulf of Mexico" as the "Gulf of Texaco"...not realizing the change would remain that way by default.
The early morning show was an outdoors/fishing show that had no weatherman aboard but the director could access weather graphics and the weather computer system kept them all up-to-date, automatically. The host of the fishing show calls for a look at the radar and when it comes up, there is "Gulf of Texaco" in large print near the middle of the image.
He spent the next five minutes of so of his show having fun with that error, and it had my department head looking at me over the top of his bifocals the next day.
