I can't remember the exact details, but it's kinda like this:

Lieberman "organized" a new party for which to run on as a candidate for Senate after loosing in the Democratic primary. He filed papers with the CT SecState and named the party "CT for Lieberman."

After winning his Senate seat, he pretty much abandoned the party on whose line he ran...he was the only person running for any office statewide on that party's line.

Shortly after Lieberman was elected, this Dr Orman guy officially switched his party affiliation from Democrat to CT for Lieberman. According to CT laws, a party convention may be called when more than xx% of the party members petition for one. Dr. Orman was the only member, so he petitioned, sent a notice to the SecState that a convention was going to be held, and that party officers were to be elected at that convention.

He was the only one who showed up at his convention, and he elected himself chairman of the party.
