When? Botox is my bet. She's in the paper every day, on the news every night. When running for President you have to press the flesh and be before the media every day. And with any such surgery, it's going to take a couple weeks for bruising and swelling to subside. When has she been out of sight for 2-3 weeks?
Do you have any idea how long it takes for facial reconstructive surgery to heal? For weeks the purple and green bruising on top of the horrible swelling makes a person look like a space alien. Then the swelling alone can make the person almost unrecognizable to their closest friends for months. A plastic surgeon will tell you that it takes a year for all the swelling to subside. Can you even think of one month when you have gone without seeing her hateful face in front of a camera?
