My maternal and paternal family are from Dallas. Good Texans are taught to say “Yes Sir” and “Yes Ma’am” to their elders, and that’s what my mother and dad drummed into me.
I think Dubya made precisely the correct greeting.

US President George W Bush drew gasps at the Vatican on Saturday by referring to Pope Benedict XVI as "sir" instead of the expected "His Holiness", pool reporters said.
A non-Catholic has no more business addressing the Pope as "your Holiness" than an American citizen does bowing or curtseying to foreign royalty.
