The benchmark for disqualification is "ajudication of mental illness," not "commitment to a mental institution".

Cho was adjudicated to he mentally ill.

NICS should have rejected his application to purchase any firearm.

What happened?

Actually, I must correct you: It appears from my research that a court-ordered commitment to a mental institution is also a disqualifier.
