My guess is there is more here than meets the eye.
Isn't there some sort of privacy for people who refuse to give up their body parts? There might be reasons that are nobody's business.
Why should the sister be put on the spot like this? Obviously there are no concerns for her. This gives a hint about why she might refuse.
