Before they open him up, they should use one of those new small intestine cameras the size of a pill. If they've got a guesstimate as to how many inches per hour it should travel and it takes forever, then it could give them a clue. Maybe a scope would work, too.
The pill would certainly be easier for the kid to...ahem...stomach.
