I think that he got a fourth star to go with his new job, and the Senate has to approve that raise, not the appointment, per se.
The vote (81-0) was to confirm his promotion to General. The Senate does confirm certain military appointments such as Chief of Staff etc.. The Senate vote on Petraeus, however, was to confirm his nomination "to General."
