He's 22 now and the article says that people who have what he has usually die in their 30s. They might have a fairly long-term customer.
Ordinarily, you can't get into Hospice unless you're terminal and a doctor thinks you have less than 6 months to live. I don't know how Terri Schiavo got in, but that's what Hospice told me when I signed my mom's rights away to them. Of course, doctors can be wrong, and Hospice won't necessarily kick a patient out if they exceed their 6 months.
I have less of a problem with prostitution than I do with abortion. Indeed, if laws against the latter are unconstitutional, laws against the former are on shaky ground. In the former case, there's no baby's life to protect (yet).
