The reason we don't have a TB vaccination program is because an effective vaccine doesn't exist. BCG, the vaccine for TB, protects to some extent against certain severe types of TB in infants and young children (e.g., TB meningitis) ages 0-5, but it doesn't protect against TB in adults and adolescents. It's only useful for preventing child sickness and death in areas where TB is highly prevalent and the risk of infection is high (and even then it's efficacy is debated).

Since our rates of TB infection are so low relatively, it doesn't make sense to have a vaccination program.

You are right in sense that we do not need a TB vaccination program!
The point I intended to make is that, lack of such program make us more vulnerable to TB infection.
The meningitis TB as well as genital TB, pleural TB, bone TB, kidney TB is due to the infection with the same bug, Mycobacterium tuberculosis.The difficulty of meningitis TB treatment reside in the place where the infection is developed.
The BCG vaccination in childhood is a lifelong protection. An adult can get infected and experienced TB, even he/she was vaccinated in cases of immunocompromission(HIV/AIDS, people under medication to prevent a transplant rejection etc.)mal-nutritons .
In US, the most vulnerable to the TB infection are the children( worry about nanny from South America, India, China..)and immunocompromissed people.
