The facts are that they are not closely inventoried by the Iraqis (nor in some cases by the Pentagon), not that they are actually "missing".
What the report identifies more than anything is lack of good organization by the Iraqi security forces. It cannot definitively say that the lack of clarity in the Iraqi record systems means that all 14,000 of the items are not in the hands of Iraqi security forces. But, that is what we are supposed to guess the report "documents", which it doesn't.

WASHINGTON (AP) -- Thousands of weapons the United States has provided Iraqi security forces cannot be accounted for, and spare parts and repair manuals are unavailable for many others, a new report to Congress says. Nice .... looks like Jihad Jimmy is gonna have brand new M-16's to play with.

There is a lot of turn over in these army and security forces. Guys take off to take their pay home and don't come back. Probably due to that.
