Well, markets are my thing, as you probably know, but I wouldn't have the first guess at the size of the cash mkt for medicine. I might very well participate as a cash player if there were some way to do a good job on price discovery.
Short of calling every physician in the book, though, I wouldn't know how to do that. (shrug)
