Whose picture? If Bush/Koizumi, then probably in flight - why would they sit and discuss something inside the plane while on the ground? Air Force 1 does not sit on tarmac for hours while waiting for takeoff permission, nor do its passengers crowd in long lines with their carry-on luggage, waiting to de-plane. The photoshopped-in guy behind the window? - most probably taken on the ground.
