That incident turned out to be the result of a botched maintenance job. After replacing an engine, the crew used the wrong fittings for the fuel line.

It was a similar, but not interchangeable bracket which permitted chafing of the fuel line by the hydraulic lines.

The pilot left the crossover valve open even when faced with evidence that there might be fuel leakage.
