It does sound exactly like pain or pane, doesn't it? I'm not sure how pain stands for drugs. Now 'cain could be for cocaine, but 'cain doesn't sound quite right either. There HAS to be some official lyrics online somewhere, but I haven't been able to find them yet... lol
