They (the nurse) wasn't imagining it, they were lying. There was no record in all of her files baring out what they said. If a person thought to be in a PVS started talking, EVERYBODY would know.

The nurse that was on record also testified that her notes would be gone the next day, after recording her observations. How convenient.

You're mistaken. There is something in her files saying that she speaks and says the word "STOP".
