PBS had a program recently about the Nazi's T4 program, where disabled people were rounded up and euthanised. Physicians who signed the euthanasia papers often didn't even interview the patient/victim, they relied on med reports sent up the chain of command.

There was also something on PBS recently, a multi-episode series on Auschwitz, which covered the early days of the T4 Nazi euthanasia program which later morphed into the concentration camps' annihilation program.
