Somewhere Dr. Gambone stated Terri was aware,she was not PVS.
Then, the MSM tried to imply Dr. Gambone stated she was PVS by saying Dr. Gambone and 5 other doctors stated that.
His name was on the PVS eval, but no signature, and he resigned as her doctor soon after.
FL_engineer needs the info I stated in the first line. Are you aware of this or can you point him to the source?
By PVS evaluation, do you mean the hospice certification form? That's what was posted here for a while -- and Gambone's name is sure enough TYPED in, but he never did sign it. It was actually signed by __Moore MD on 4/11/00. The referral was taken by Catherine C. Hodge, GS and it was signed by ... can't read it well ... _____ndy Cusimano RN. It was done on 3/3/00 life expectancy less than 6 months; terminal diagnosis WRITTEN IN in this otherwise typed form): vegetative state.
In the bottom typed in it says: "Based on the patient's diagnosis and current condition, I expect this patient has a limited life expectancy of six (6) months or less, if disease continues to take its usual course, and hereby certify this patient as eligible for Hospice care." Then is the place for physician to sign as the one verbally certifying this info is true. Gambone's name is typed in, but Moore actually signed.
