I believe it is California where some ballots are just taken out of their envelopes and the ballot thrown in a pile and the envelope discarded so there is no way to determine where these types of ballots come from...I think they were provisional or absentee...

Incorrect. Nothing is discarded in the absentee process in California. Theouter envelopes are sorted and saved for three years.
