At the end of the questioning process, the potential donee has a private opportunity to place a bar-coded sticker on the questionaire that indicates whether their blood should be used or not used.

The Red Cross has been doing this for years.

I've donated over 12 gallons of blood and 15 gallons of platelets over the years.

Agreed. And congrats! I'm still working on my 5th gallon. O- blood type.

To answer the question, it would only be a violation if he put the "use my blood" barcode sticker on his form.
