I reviewed a 35MB .AVI file of the incident and this is the sequence of events:

Pulls out
Either unfolds and places or just places it, difficult to tell. Perhaps he flipped it (turned over)
Looks at it, looks at Lehrer
Reaches to the right
Uncaps a pen
Holds the pen up and begins to write

The clip I saw just showed his hand in his jacket pocket and making a turn to the podium.
