X-15 broke up due to improper re-entry attitude (like almost backwards). One broke in the middle on the ground due to a lousy landing. I've got the video around here somewhere.

That's right, that was a PIO wasn't it? That would be a control problem.
They also had one blow up on the engine test stand. The pilot in the cockpit (Crossfield?) claims fasted man from 0-60 feet.

That X-15 landing (cracked in the middle) was the crash at Edwards AFB that they used in the 6 million dollar man TV show intro.
