I found the following at the Air Force web site

The regulation is obsolete and is not available online. A FOIA request could take weeks.
When I last used it in 1990, it did indeed say what it is quoted as saying. Nevertheless, failure to take a flight physical has many different reasons. I missed mine in 1990 because I was in f*cking Saudi Arabia dodging SCUDs.

37. Addition.
A Google search of the phrase "officer effectiveness report" returns over 400 results. A search of "officer effectiveness training report" returns only 10. And every last one of them has to do with George W. Bush supposedly being AWOL from the Texas Air National Guard.
Separately, a Google search for OETR + Air Force (excluding "Kerry") lists only 9 results, and every one of those sites refers to the forged documents, using Officer Effectiveness Training Report as well.
The only people using OETR are those associated with these forgeries.

Correction:
The OETR is the Air Force Officer Education Transcript Repository at Wright-Patterson AFB.
The only references ever calling it an Officer Effectiveness Training Report are those linked to this forgery.

Yep. A routine scheduling order would have been IAW AFM 160-1
Judicial proceedings after failure to appear would have been IAW AFM 35-13.

This is quoted from teh Kerry-Edwards, April 27, press release:
"Bush Was Suspended From Flight Duty For Failing To Take Mandated Medical Exam.
On September 29, 1972, Bush was officially suspended from flying for missing his annual medical examination. The orders note that Bushs suspension is authorized under the guidelines presented in Air Force Manual 35-12 Para 2-29m, which reads that Bushs local commander will direct an investigation as to why the individual failed to accomplish the medical examination. [Aeronautical Orders, Number 87, 29 Sept 72; AFM 35-13, Para 2-29m] Where Are The Complete Results Of The Required Investigation Into Bushs Absence From The Exam?
FACT: The order suspending Bush from flight duty stated: Verbal orders of the Comdr on 1 Aug 72 suspending 1STLT George W. Bush
from flying status are confirmed
Reason for Suspension: Failure to accomplish annual medical examination. Off will comply with para 2-10, AFM 35-13. Authority: Para 2-29m, AFM 35-13. [Aeronautical Orders, Number 87, 29 September 1972, emphasis added]
Para 2-29m, AFM 35-13: When a Rated Officer Fails To Accomplish a Medical Examination Prescribed by AFM 160-1
(1)The local commander who has authority to convene a Flying Evaluation Board will direct an investigation as to why the individual failed to accomplish the medical examination. After reviewing the findings of the investigation, the local commander may convene a Flying Evaluation Board or forward through command channels a detailed report of the circumstances which resulted in the officers failure to accomplish a medical examination, along with a recommendation that the suspension be removed. (2) The individuals major command will forward the report along with the command recommendation to USAFMPC/DPMAJD, Randolph AFB TX 78148 for final determination. [Para 2-29m, AFM 35-13, emphasis added]
And that's the ONLY reference ANYWHERE where I have found any information about the AFM 35-13.
(PS. I agree that AFM 35-13 is "probably" a valid manual, since it is used in the order removing Lt Bush from flight status, and referenced in a flight status change for a group of air crewmen in the 1950's) but that's it. )
