The incident was on Feb 28/69 I believe. The second citation was issued sometime between then and Dec. 5, 1970, when Hyland no longer was commander in chief of the Pacific Fleet, according to this site:

Correct. Hyland was CINCPAC from 30 November 1967 - 5 Dec 1970. ADM Zumwalt was Commander, US Naval Forces, Vietnam from September 1968 - May 1970. Both of them were in their positions to pass on Kerry's Silver Star, which was awarded for action on Feb 28, 1969. We have photographic evidence that Kerry received his Silver Star in Vietnam.
