Cleland lost his limbs in a non-combat related accident. Regrettable to be sure, but it had nothing to do with combat.

He saw a grenade on the ground and thinking it was one of his, he picked it up. He was not in combat but on a base.
But he looks pitiful.
He did not get a purple heart because it was self inflected.

You are correct. Cleland was not wounded in combat. He, or someone on the same helicopter, was careless with a grenade. That is why he wasn't awarded the Purple Heart for his injurys.
