The only shrapnel Kerry had was that little piece, which was removed by hand, and closed with a bandaid according to Dr. Stetson, who treated him. The rice, which was stuck in his a** was self inflicted, and also removed by hand.

apparently he does have a very tiny piece of something in his leg - according to x-rays (source Boston Herald late april or early may '04) it measures about 2mmx2mm - which must mean it's even thinner although senator booboo has claimed it "caused him pain for years"
