A chief resident is most likely a fourth year resident with extra responsibilities above and beyond those of the other residents. The income is low, as she is essentially just an apprentice. They are having to pay medical school costs and are probably both under extreme pressure with no time to think about taking care of an apartment, much less a house.

My guess is that the "dormitory" was more like married student housing for residents.

Residents don't begin to make money until they finish their residency. They are doctors in servitude until then.
