He was the doctor's corpman, i.e. the guy who would do the paperwork. I believe that his name was something like Correa.

Yes, the corpsman is dead (the one who signed the report). The Doctor who gave the exam is still alive.

His name was J.C. Carreon. He was a Hosptial Corpsman (HN) and worked for Dr. Letson. He died in 1992.
