All first-time customers must show proof of their residence address in North Carolina. This is in addition to showing proof of identity. If one of the two forms of proof of identity also includes the customerís current North Carolina residence address, that is sufficient.
Acceptable proof of residency documents include:
1. Any document issued by the State of North Carolina or any of its political subdivisions (county, city, tax district, etc.) or by the federal government
2. North Carolina voter registration card
3. Preprinted bank or other corporate statement
4. Correspondence on preprinted business letterhead
5. Apartment lease, housing contract, mortgage statement
6. Utility bill (power bill, cable bill, water bill, etc.) or contract for utility service
7. School records
8. Matricula Consular issued by the Mexican consulate
9. Letter from homeless shelter
A sworn affidavit may be used ONLY if one of the above documents is NOT available. In the case of a minor, a parent or legal guardian must sign the affidavit. General Statutes requires a $2.00 fee for execution of an affidavit.
