I helped with a number of child porn Internet cases... yes, there is a commercial market centered around new or extreme material not already freely available in newsgroups.
Just two years ago there was an major case involving a Russian child porn company called Blue Orchid. Individuals in several countries who had bought the videos were also arrested, including many Americans.
