That's why the outfit was specially made with a tear-away section and Janet just happen to have a pastie underneath. Completely 'unplanned'. Upon reading about this and knowing MTV's reputation and seeing the rapid societal decay I initially believed that this was intentional. However, after seeing an HDTV screen capture and viewing a video clip I'm not so absolutely sure.
The existence of the pastie was damning. But, in fact, it wasn't a pastie. It was a sunburst shaped piece of jewelry attached to Janet's nipple through a nipple pierce. Most of her nipple was exposed. See the hi-res pic.
Also, her reaction looked genuine - like something went wrong. And I know she's a tramp, and I think she's fully capable of a stunt like this. But, even to my jaded eye, her reaction looked genuine.
These observations certainly don't conclusively prove innocence, especially given some of the other evidence that this was pre-meditated, they're just some thing I noticed.

I have done a lot of sewing in my day. And I have never yet sewn an outfit that specified velcro attached bra sections. Nor have I ever seen such an outfit while shopping (but maybe I don't shop at the right places). Dancers are especially particular about their costumes being securely sewn because the movements they do can easily bust a seam when and where they don't want one busted.
I don't for a minute buy that this wasn't purposeful. How did Justin know exactly where to pull to remove the section? He reached for the exact spot that would cause it to happen--under her right armpit--and pulled it toward him. The shock on Janet's face was feigned.

She's an actress. Maybe not a great one, but good enough to pretend to be "surprised" which is not exactly one of the more subtle emotions.
