It is a "Nipple Shield" Held in place by a piercing that runs through the nipple. Its a style called a "Starburst" design. They are worn to increase stimulation of nipple--or so that's what people say.

When I posted my "pastie" comment last night, the close-up on Drudge was not yet available. I wuz just callin' it as I saw it (then).

I have since seen the close-up, and all I can say is, "Ouch."
