I'm also puzzled as to how to prevent this sort of thing. Short of mandating a complete gynecological/rectal exam for every passenger, wouldn't those new X-ray-type scanners detect this kind of foreign substance inside a person's body?

Diagnostic x-rays would be able to detect plastics explosives in the vagina, but they are impractical.
If dogs can be trained to detect drug smugglers, I am sure they can do the same with explosives.
